What will happen on my first visit?
- A thorough examination will be completed by Dr. Herriges. After the exam, Dr. Herriges will
let you know if he recommends any treatment and answer your questions. - Patients will receive an estimate of treatment fees and out-of-pocket costs.
- Any treatment will be scheduled for another day.
- A referral and any recent pertinent x-rays from the referring office will be needed prior to
the first appointment. - Dental insurance information is also requested prior to the first visit so we can obtain your
plan information. - Please bring a list of medications to assist in filling out the health history form.
Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints requiring antibiotic pre-medication, severe diabetes, or hypertension).
